Sister Sage has likely been lobotomized in The Boys S04E03. The biggest clue (besides a lobotomy tool shown at the end of the scene) is the total change in her behavior. She’s gone from being calculating and distant to friendly.
Before, she obviously looked down on The Deep, and now she’s suddenly treating him like an old friend and even having sex with him. She’s also is eating junk food and watching “low-brow” movies like Transformers 2. Obviously, something has happened to change her behavior.

The big question is whether this was done to her or if she did it to herself. Fans theorize it’s something she does recreationally. Being the smartest person in the world is a pretty big burden, and the one way to escape the pressure for a while would be to literally destroy part of her brain.
The missing piece to the puzzle here is hidden in Sister Sage’s profile, which is seen on the screen in the Seven’s meeting room. It shows that one of her “Special Skills” is moderate regeneration. So, she can regenerate damage, but it takes more time than a supe like Kimiko. So, she can lobotomize herself and relax for the few hours it takes for the damage to heal.
